Required Documents

We require several documents to be completed prior to your appointment. This includes a single Financial Agreement followed by these four Personal/Medical Documents:

  • Agency Disclosure Notice
  • Personal Emergency and Disaster Plan
  • Personal Information and Medical History
  • Consumer Rights Notice

Click on the button below to complete these online. Each document will appear with instructions at the top. When you have completed a form, click on the Submit button and the next form will appear. Repeat the process for all five documents. NOTE: it can take some time to process between each form. Please be patient and do not navigate away from the page.

When all documents have been completed, a Thank You screen will appear. You will receive a separate email, with your completed document attached, for each form you fill out.

Print Option: Click on the link below to print the Financial Agreements (select the one that best fits your financial situation) and Personal/Medical Documents. Please fill them out and bring to your first appointment.
